Crescent Health Care (CHC) is a Skilled Nursing Facility that is locally owned & operated. We are located in beautiful, residential West Yakima convenient to physician's offices, hospitals, and transportation services. Your first impression of Crescent Health Care will be one of spacious, open living areas. The two outdoor patios and walkways are often utilized by residents and their family and friends. Not only is Crescent Health Care locally owned and operated, we have the distinction of being one of the few skilled nursing facilities with stable, long-term management staff. All of our dept. supervisors have been with CHC for over 10 years. Some of the Services offered at CHC are Nursing Services, Skilled Therapy, Rehabilitation (short & long term), Activities, Social Services, Respite Care, Dining Services and many, many more. Crescent Health Care is capable of providing nursing care that previously was only provided in an acute hospital setting. We accept Medicare, Medicaid, Veterans and most insurance. We also have staff on site to assist residents and thier families to navigate through these complicated agencies and receive the benefits to which they are entitled. Next door to CHC is Crescent Place (CP). Crescent Place is a small retirement community that provides independent living w/a combo. of personal services. We pride ourselves on our ''family '' atmosphere and the special attention we are able to offer in our small facility. Out of town family or friends visiting a loved one at CHC or CP have the opportunity to stay in a suite at CP for a nominal fee.
